Saving the Form

In order to save the form, the file type and form method combination must be unique. In other words, a submission form can only match up with one management form. If multiple management forms existed for Patient Accidents, for example, RL6 would not know which management was the correct one to use. If you attempt to save a form where the combination already exists, an error message will display.

Prerequisites:The 'FormsClosedA number of fields grouped together by sections to allow users to enter and store data related to a patient or employee event. You can define multiple forms for a module. Forms fall into two categories: submission forms (i.e. for front line entry users) and management forms (i.e. for file manager users). Management forms contain the same data as submission forms with added sections for follow-up investigations and resolutions and outcomes.' role function must be enabled.

  1. Create or open a form.
  2. Click Save. The Save Form dialog appears.
  3. Note: the form must contain the General EventClosedA patient or employee related occurrence which is recorded in paper or electronic format. Once recorded, an event file is reviewed by a file manager who may involve other individuals (e.g. using tasks and follow-ups features) to help investigate and resolve the event. Type, Last Name, First Name, and File Name fields in order to be saved.
  4. Complete the following fields:
    1. Form Name – the name of the form as it will be stored in the database.
    2. File Type – the values that appear here are defined in the IncidentClosedSee Event Type pick list.
    3. Form Method – either submission (SUBMIT) or management (MANAGE). The combination of this field and the File Type field must be unique.
  5. (For Submission forms only) Select Check for duplicates to have all files of this form included in a duplicate check (See Duplicate Management Overview for more information about this feature).
  6. Select Archived to archive the form. Archived forms do not appear in the Form Designer by default.
  7. Note: archiving a form does not affect its use elsewhere in the system. For example, users can still submit files using archived forms associated with icons on the Icon Wall.
  8. Click Save to save your form in the database.
