Patient Field Descriptions

The following table describes the various fields shown in the New Patient dialog box:

	Title	N	Job title.
	Preferred Name	N	Preferred name, if applicable.
	Suffix	N	Additional information, e.g., academic, honorary or professional suffixes.
	Sex	N	Male or female.
	Birth Date	N	Date of birth.
	Birth Date Precision	N	Precision of birth date.
	Deceased	N	If member is deceased.
	Deceased Date	N	Date of decease.
	Deceased Precision	N	Precision of date of decease.
	Person Type	Y	Patient.
	Home Phone	N	Home phone number.
	Business Phone	N	Business phone number.
	Fax	N	Fax number.
	Cell Phone	N	Cell phone number.
	Email	N	Email address.
	Ethnic Origin	N	Ethnicity of the patient.
	Marital Status	N	Marital status of the patient.
	Language	N	Primary language of the patient.
	Race	N	Race of the patient.
	Religion	N	Religion of the patient.
	Veteran ID	N	Veteran ID number of the patient.
	Veteran Status	N	Veteran status of the patient.

	Birth Weight	N	Birth weight.
	MessageThe test results are received via a message from the client integration engine to the RL integration engine. Each message may contain one or more test results; these test results only pertain to a single order. Source System	N	Source system of the message providing details of this patient (from ADTAdmission, Discharge and Transfer data. feed).
	Message Source Date	N	Date/time of the message providing details of this patient (from ADT feed).
	Message Source ID	N	Identifier of the message providing details of this patient (from ADT feed).
New Patient Identifier	ID	Y	Unique identifier.
	Assigning Authority	N	Assigning authority for the patient identifier (from ADT feed). This is the facility/site from where the ID originates.
	Type	N	Identifier type, e.g., Driver's License, Passport, etc.
	Facility	N	Home facility.
	ID Status	N	Indicates if this patient identifier has been merged (from ADT feed), i.e., Merged or Linked.
